Anamnesis assistant · Built into Fisiotrack
Stop writing up the clinical history. Listen to your patient.
ONA Scribe sits in on your session, understands what is said and writes the structured anamnesis straight into the patient record. You review, sign and move on.
No credit card required. After that, €19.95/month + VAT for the whole clinic.
In the treatment room
PATIENT I've had pain in my lower right back for three weeks. It started after I moved house.
PHYSIO Does the pain go down your leg?
PATIENT Down the back of my thigh, but not past the knee. It's worst in the morning, about a seven out of ten.
PHYSIO Let's test trunk flexion.
Anamnesis · ONA draft
- Reason for visit
- Right-sided low back pain of 3 weeks' duration, onset after a lifting strain (house move). 00:12
- Pain profile
- Radiates to the posterior thigh, not beyond the knee. Worse in the morning. VAS 7/10 at worst. 01:04
- Functional assessment
- Trunk flexion limited, pain at end of range. 03:48
- Hypothesis
- Mechanical low back pain with a somatic referred component. Rule out radicular involvement.
- Treatment plan
- Manual therapy + motor control programme. Reassess in 2 weeks.
The problem
The clinical history gets written twice: once in your head and again at nine at night.
A well-documented initial assessment means fifteen or twenty minutes of writing. Multiply that by every visit in your week.
It comes out of your patient's time
Every minute in front of the screen is a minute you are not watching, assessing or explaining. And the patient notices.
It's written from memory
Notes written hours later lose the nuance: the patient's exact words, the detail that seemed minor and turned out to explain everything.
And it has to hold up
An incomplete clinical record is a problem if it is ever requested. Documentation is not red tape: it is your protection.
How it works
Three steps. The last one takes ten seconds.
ONA Scribe doesn't change the way you work: it fits on top of the session you already run.
- STEP 01
Press record and treat
Open the patient record in Fisiotrack and hit record. From there, run the session as usual. ONA listens in the background, on your phone or your computer.
- STEP 02
ONA writes while you talk
It doesn't transcribe: it interprets. It separates what the patient says from what you say, tells the history apart from the examination findings and puts each item in its field.
- STEP 03
Review and save
When you finish, the draft is right in front of you. Every sentence links to the moment in the conversation where it was said, so checking means reading, not reconstructing. Correct whatever you like and save.
Privacy
Patient data stays where it belongs
Processing on servers in the European Union, encryption in transit and at rest, and the audio is discarded as soon as the note is written. Your patients' conversations never train any model.
Time
Documentation is no longer after-hours work
The note is ready while the patient is still putting their coat on. Reviewing it takes a minute because every item carries its timestamp: you don't have to trust it, you can check it.
Integration
It writes in the patient record, not in a separate document
ONA Scribe lives inside Fisiotrack. The note lands in the patient's history, alongside the diary, prescribed exercises, signed documents and Verifactu invoicing.
What it fills in
A physiotherapy anamnesis, not a generic summary.
ONA Scribe is built on the structure physiotherapists use to document, with the fields that already exist in the Fisiotrack patient record.
- Reason for visit and historyWhat brings them in, since when, what triggered it and what they have tried before.
- Pain profileLocation, radiation, pattern, aggravating and easing factors, VAS score.
- Assessment and functional testingThe tests you call out aloud and what you observe, captured as you say them.
- Hypothesis and clinical impressionA suggested line of reasoning, always as a draft. The decision is yours.
- Treatment plan and reassessmentLinked to the exercises you prescribe in Fisiotrack and to the next appointment.
Follow-up session · 12 March
- Progress
- Reports improvement in morning pain, VAS 4/10 versus 7/10 at the initial assessment. 00:38
- Adherence
- Completed the home programme 5 out of 7 days. Reports difficulty with the quadruped exercise. 01:55
- Functional assessment
- Full trunk flexion, mild discomfort at end of range. No distal radiation. 04:20
- Treatment plan
- Progress loading. Replace the quadruped exercise with a standing variation.
Follow-up visits too: ONA compares with the previous session and records progress.
Security and compliance
You are recording a patient. That means doing it right.
You handle special category data. ONA Scribe has been built to take on that responsibility from day one.
Encryption and EU hosting
Audio and notes are encrypted in transit and at rest, on infrastructure located in the European Union.
The audio is discarded
As soon as the note is written, the recording is deleted. What is kept is the clinical record, as always.
We don't train on your patients
Conversations from your clinic are never used to train or improve models, neither ours nor anyone else's.
Built-in consent
Informed consent for recording is collected and signed digitally from the patient record itself, and filed with the rest of the documents.
Every field is traceable
Every item in the note points to the moment in the conversation it came from. Nothing appears without you being able to check where it came from.
Always a draft
ONA suggests, you validate. No note enters the clinical record until a registered practitioner has reviewed and saved it.
Pricing

It's an add-on to your plan
ONA Scribe requires a Fisiotrack account: that's where the patient record it writes in lives. The €19.95 is added to what you already pay for the CRM.
Three practitioners are included in the base price
If there are two or three of you, you pay nothing more. A clinic with six practitioners would pay €19.95 plus €15: €34.95 a month for the whole team.
Fifteen minutes per initial assessment
If you document four initial assessments a week, that's well over an hour of writing. ONA costs what that hour is worth spent on anything else.
Who builds it
ONA Scribe was born inside Fisiotrack
The practice management software already used by hundreds of physiotherapists in Spain, designed by physiotherapists.
It is not a generic management app, you can tell it is specifically designed for physiotherapists.
